Retinoid Ladder for Indian Skin: Starting Without Burning

9 min read J. Drugs Dermatology (2023) · 450 subjects · Fitzpatrick III–V · 24 weeks

Retinoids remain the gold standard for photoageing, acne, and skin texture — with the deepest evidence base of any topical skincare ingredient. But the standard "start at 0.1%, apply nightly" advice was developed for European skin and causes significantly higher irritation in darker tones.

The Indian retinoid irritation paradox

Fitzpatrick III–V skin has a higher epidermal melanin density that provides some UV protection — but also means barrier disruption from retinoid-induced desquamation is less visible until it becomes severe. Indian users frequently over-apply retinoids without the visible redness that would prompt a European user to reduce frequency.

The validated ladder approach

Week 1–4: Granactive retinoid 0.025% twice weekly. Week 5–8: Granactive retinoid 0.05% every other night. Week 9–16: Retinol 0.1% nightly. Week 17–24: Retinol 0.3% or prescription tretinoin 0.025% if needed. This 24-week approach showed 70% lower barrier disruption and equivalent photoageing reversal at 24 weeks vs. aggressive start protocols.
